How to use the PHP network:

  • Select a physician from the directory.
  • Call your selected provider to schedule an appointment.
  • Identify yourself as a PHP participant.
  • Verify that the provider is an active participating member of the PHP network by calling 1-800-474-6323.
  • Have a copy of your current insurance identification card with you. You will be asked to show your ID card each time you visit a provider.
