Reimbursement information:
PHP participating providers may request information from any plan’s fee schedule
listed above by calling 229-312-8061. New providers that supply PHP a list of their
top 30 CPT codes during the opt-in process are given a Fee Schedule Report. All
fee schedule information given to providers is proprietary and confidential. Dissemination,
distribution, or copying of fee schedule information to any person or organization,
for any purpose, without the written consent of Phoebe Health Partners is strictly
prohibited. PHP will not provide any opinion regarding the fee schedule(s) that
it forwards to the provider.
